ConnectiCare Medicare Advantage transition policy
For Medicare Advantage members who may be affected by a formulary change or members who are new to ConnectiCare, we want to make sure your transition is as seamless as possible. That’s why we have policies in place for any prescription drugs that you are taking.
We have taken steps to ensure that prescription drugs are used safely and effectively by our Medicare Advantage members.
We perform quality assurance reviews to reduce and prevent medication errors and adverse drug reactions and to improve overall medication use.
Best available evidence
The Centers for Medicare & Medicaid Services (CMS) created this policy that requires sponsors, like ConnectiCare, to establish the appropriate cost-sharing for low-income beneficiaries when presented with evidence that the beneficiary’s information is not accurate.
